Prepare for your first appointment
To make your appointment progress faster, please print and fill out the Patient Verification Form and the Patient History Form ahead
of time.
please bring the following
- Your filled out forms
- A current photo ID
- Your insurance card
- Any referral or authorization documents as required by your insurance provider
- Any laboratory test results for the past 6 months
- A list of all your medications and supplements by name, including dosage and frequency
- You may also bring your prescription bottles and supplements in a bag with you
- A list of all current doctors, including address and phone number
- Be prepared to share your medical and family history
Please arrive at least 15 minutes prior to your appointment so we may verify your paperwork.
